The foundation for health promotion was established in June 2004 as an initiative of people from CAN-PACK S.A. in Krakowie. The idea was to give social aid, promote and care for health. Founders and establishers of the foundation are:
Mr. Wojciech Burczyk, Mr. Jerzy Sarna, Mr. Wiesław Smulski, Mr. Stanisław Waśko and POL-AM-PACK S.A. Company.

The foundation has been registered by Regional Court for Kraków -Śródmieście on the 21st of June 2004 and entered into National Court Register under the number 0000210386 (files number KR XI NS-REJ.KRS/10170/04/367).
